In the next few posts, I'm going to re-focus on the Patient-Centered Life-Cycle Value Chain series [click here for the first post in the series]. Specifically, I'll discuss how to bring high value to the healthcare consumer through a new type of wellness program we're developing, which offers a whole-person integrated-care solution. I welcome your questions and comments. What is Whole-Person Integrated Care?As the name implies, whole-person integrated-care has two related parts: Whole-Person care and Integrated care.Whole-Person CareWhole-person care focuses on improving a person's health and wellbeing by addressing one's physical health (body), mental/psychological health (mind), and the mind-body connection ("biopsychosocial" health). In other words, it views an individual as a whole entity, whose body and mind are interconnected. The whole-person perspective is critical for preventing and treating health problems in a cost-effective manner because it helps lower overall healthcare e
LONDON: Indian Tamarai has blossomed in Britain -- the popular restaurant chain added another feather to its reputation for culinary excellence, this time by winning an award for the Best Night Bar in London.Tamarai (meaning lotus in Tamil), India's Old World Hospitality group's third London restaurant after Chor Bizarre in Mayfair and Sitaaray, has got the accolade at the Theme Bar Awards 2008. The award by Britain's widely read magazine, Theme, celebrates the best in the industry bringing together excellence in modern drinks, design and hospitality in different categories. "lt's an exceptional honour for a bar operated by an Indian company in the most competitive food and drinks city in the world," said a spokesperson for the group which operates the Habitat World at India Habitat Centre as well as 12 other restaurants in Delhi. The restaurant was last year awarded 'The Best Wine List in London' by 'The Independent' newspaper and 'The Best Pan-Fusion Food in London' by the 'Evening

Here's an alarming trend: tea bags are now being called "t sacs." It's bad enough that I have to hear about "teabagging," God only knows what they're going to come up with for "t sac'ing".....Speaking of tea bagging, I was watching a show on E! (gotta get that ! in there to make it legit) about prisoners and the strange folk who write to them. There is a website for male and female prisoners to post their pictures and bios. Sort of an eHarmony for the criminally insane, if you will. Anywho, this one prisoner was talking about the things he wrote to scam--I mean, to attract women. First of all, he talked about why he was in prison. He said he and his friend were confronted by another known person and they started fighting. In the meantime, the prisoner ran to get his gun because that's what one would normally do in an altercation and when it seemed like the third person was going to shoot them, he "felt like he had no choice" but to shoot first. Apparently the jury who tossed his ass in

Having affordable insurance and comprehensive coverage is a cornerstone of the PCLC Value Chain. Nevertheless, one of the issues generating the greatest debate concerns universal health coverage (i.e., healthcare for all), along with the related issue of "personal responsibility." The argument tends to center on whether the young and the healthy should help pay for care that older and ill persons need, how much they should pay, and the degree of responsibility people have for staying healthy and paying for their care when they get sick. In an earlier series of posts, I made the case that all Americans are worthy of health insurance, and that personal responsibility is a complex issue we must address, but for which there are no quick fixes.Well, I recently corresponded with two individuals who have ideas about this topic, which I present below.Person 1 wrote: Just an idea, but since some here are so determined that Health Care for Everyone should be equal (but not equally paid for) and
Not only does 2008 hold out the prospect of new stuff from the Valentines, but that other candy-and-barbed-wire outfit, the Jesus and Mary Chain, will also be flinging out a new album:The siblings are now whittling down dozens of new songs to 12 or so for the new album. They performed one track, "All Things Must Pass," this summer on "Late Show With David Letterman," and will appear early next year on the soundtrack to the hit NBC drama "Heroes." Another, "Dead End Kids," has been a fixture in the band's recent live sets."I would say it's an evolution," [William] Reid says of the new material, which he and his brother wrote separately. "It definitely sounds like the Mary Chain, but I guess you evolve as a person and a writer. You can't really stand still. If you do that, you're lost."Here's that Letterman appearance, to help you decide if this is good news, or just last knockings:

Continuing the series on the PCLC Value Chain strategy [click here for the first post in the series], this post focuses on a third process in need of reform: The transformation from "pay for performance" to a "pay for value" model that rewards providers who deliver high-value to patients/consumers. Being a complex and controversial issue, this post is lengthy, but hopefully insightful.Defining Pay for Performance Today's Pay-for-Performance (P4P) model uses "quality" measures (metrics)--devised by insurers, government and big healthcare institutions--to judge the performance of providers (hospitals and clinicians) and pay them based on their performance. In the P4P process, monetary rewards are given to providers who follow specific evidence-based guidelines for particular types of patients and, for some conditions, who achieve certain results in patient health and well-being. And recently, the Centers for Medicare and Medicaid Services (CMS) decided to add a "stick" to the P4P "carrot
Continuing the series on the PCLC Value Chain strategy [click here for the first post in the series], this post focuses on a second clinical process in need of reform, that is, the transformation to "value-based competition."Current Competition Models are MisdirectedHealthcare market forces in America over the past decade have transitioned from managed care and capitation to integrated delivery (integration of health insurance with provider systems) to a vision in which providers compete to improve care quality and control costs, and consumers choose the best providers.[1]There is now debate on whether competition should be redirected by:Eliminating provider networks and encouraging informed, financially responsible consumers to choose the best provider for each conditionEncouraging integrated delivery systems with incentives for teams of professionals to provide coordinated, efficient, evidence-based care, supported by state-of-the-art information technologyBasing selection of particu
In my previous posts, I've discussed the benefit of integrating the three macro processes of the PCLC Value Chain strategy: emergency-care, sick-care & well-care. In my next series of posts, I'm going to focus on our healthcare system's problematic clinical and financial processes. I'll be examining ways to reform these processes so they enable the PCLC Value Chain strategy to succeed in increasing increase healthcare effectiveness, efficiency and access. I begin with the transformation to "patient-centered" care.Patient-Centered CareTo define patient-centered care, let's start by contrasting it with the kind of non-patient-centered care we have today. We currently have a fragmented healthcare system in which:Plans of care are typically one-size-fits-all, which means clinical guidelines are rarely tailored to a patient's particular needs an preferencesThe vast majority of practitioners use paper-based patient records or electronic records that are not interoperable, which means sha
In my previous post, I discussed the second link in the Patient-Centered Life-Cycle (PCLC) Value Chain, which is sick-care. In this post, I discuss the third link, which involves well-care, as well as the integration of emergency-care, sick-care and well-care.Well-Care (Prevention & Self-Management)The third link in the PCLC Value Chain involves well-care, which includes prevention & self-management. Three well-care delivery models are the: Preventative Maintenance model, which focuses on (a) delivery of primary prevention -- such as physical activity, nutrition, stress relief, vaccinations, etc. -- that help people avoid health problems, as well as promoting peek performance, and (b) secondary prevention for at-risk persons to prevent recurrences of health problems, such as avoiding recurrent coronary artery disease events in a person with a history the illness. Recovery/Rehabilitation model, which focuses on adherence (compliance) to doctors’ orders to foster recovery, reha
In my previous post, I discussed the first link in the Patient-Centered Life-Cycle (PCLC) Value Chain, which is emergency care through emergency management. In this post, I discuss the second link, which involves sick-care, also known as the “clinical encounter†between a patient and one or more healthcare providers. Sick-care consists of the following six categories of physical and psychological problems (including diseases, illnesses, dysfunctions, and traumas): Acute health problems characterized by sudden onset and short duration, which progresses rapidly and require urgent care. An acute myocardial infarction (heart attack) and serious accident victim are examples.Subacute health problem distinguished by abrupt onset, but it has longer duration or changes less rapidly than acute problems. Examples include post-operative care, complex wound management, and rehabilitation for stroke.Chronic health problem of indefinite duration, which may persi
In my last post, A Path to Profound Healthcare Transformation, I discussed how our low-value healthcare system is built upon our country’s pathologically mutated form of capitalism. I then described a new direction for transforming the current broken system into a high-value patient-centered system that delivers top quality care and does it efficiently and at a good price (i.e., cost-effectively) over a person’s entire lifetime. This past week I came across two more articles that support my call for value to the patient. The first one, Healthcare 2015: Win-win or lose-lose?, reported that:Healthcare is in crisis…[and] will become unsustainable by 2015…Value is in the eye of the purchaser, but today value in healthcare is difficult to see. Data regarding the healthcare price is tightly held and difficult, if not impossible, to access or comprehend; quality data is scarcer still and mostly anecdotal or incomprehensible. To complicate matters, the purchasers and benefactors of healt

Ive just noticed this a while ago while checking out MSN. Its very seldom that i use it but the ads below the window really gets my attention and when i got to use it earlier. Low and behold. I noticed this very familiar tagline."Crispylicious, Juicylicious CHIKENJOY!" My initial reaction was, "Are you kidding me?!? Jolibee on MSN Messenger?". Ive got no choice but to click and find out for myself. And there it was, my intuition didnt fail me. (It never fails me.hehe) I saw the familiar mascot of Jolibee Himself. And i was taken to the home page.I could only stand in awe and admiration to this company. Ever since i was a child, i never even gave Mcdonalds a chance against them. I always love the chickenjoy and the swirly bitz. This is another reason that my blog is named as such. Kirbitz and Swirly Bitz-- Does all the difference! For all of you guys who dont know Jollibee, its the number one food chain in the Philippines and well, Mcdonalds is not far behind. I have to pick my spots when it comes to saying anything about Willard Mitt Romney, I know; he just provides so much nonsense to refute that I can’t possibly answer all of it.Instead of refuting his idiotic criticism of the tax proposals of John Edwards in this story, though (a “new bike”?), I want to take issue with what he said about illegal immigration, to wit…"I ... don't think it makes sense to have an immigration policy that says that if an illegal couple — a couple that comes across the border illegally — has a child here, that child becomes a U.S. citizen, that then the whole family gets to come in, if you will, through 'chain migration'," he said.I conducted numerous searches using the terms “chain” and “migration” in the context of illegal immigration into this country, and I found nothing, which isn’t surprising (the term actually reminds me of that scene in the Woody Allen movie "Take The Money And Run" where the members of the work gang are all try
Here’s something we don’t hear about often. The Odeon cinema chain in the UK has pulled an advertisement that was being screened before all films as part of the search effort for Madeleine McCann, the four year old that disappeared from her parent’s hotel room in Portugal.
Apparently the announcements were pulled after parents complained about the advert, which showed a picture of Madeleine and describing how she was taken. I haven’t seen the advert, so I can’t speak to more details than are available in the Variety story but I wonder if there’s more to this than meets the eye.
To me, it sounds as though the theater was trying to do a ‘good deed’ and has been chided for it. Apparently parents have a problem with having to explain that bad things happen to little kids and though on the one hand I can’t say I blame them, on the other I’m confused as to why parents would have a problem with a campaign aimed at finding a lost child. Would they not want the cinema to d
